Endoscopic management of malignant pancreatic tumor

Introduction
Malignant pancreatic tumors are nowadays effectively managed by endoscopic retrograde cholangiopancreatography (ERCP) and endoscopic ultrasound (EUS) approaches. Among them, EUS-guided pancreatic tumor management is relatively new field and there has been marked advancement during last decade.
EUS-guided tumor ablation
EUS-guided direct tumor ablation is also tried by various methods such as EUS-guided radiofrequency ablation (EUS-RFA) or cryotherapy. EUS-RFA is actively investigated to several pancreatic tumors such as neuroendocrine tumors, solid pseudopapillary tumors and pancreatic cancers. Technical feasibility and clinical response rate seem to be higher than 90% and 80%, respectively. There can be some complications but reported complications are not frequent and severe. Therefore treatment response seems to be good based on several pilot studies.
Conclusion
EUS-guided management of malignant pancreatic tumors are making rapid progress. EUS-guided direct tumor ablation became possible. Although initial studies show relatively good response and safety, more studies are required to increase the efficacy and reduce possible complications.
