The Efficacy of Aescin in Combination with Micronized Purified Flavonoid Fraction (MPFF) in the control of bleeding and prevent recurrence of acute internal hemorrhoids, A randomized Controlled Trial Travel Award
Introduction: This study assesses the effectiveness of combining Aescin with Micronized Purified Flavonoid Fraction (MPFF) for treating acute internal hemorrhoids, which significantly affect quality of life through symptoms like pain and bleeding. While MPFF is effective, recurrence remains a challenge. Aescin, derived from horse chestnut seeds, may enhance MPFF's effects due to its anti-inflammatory and venotonic properties. Method: In this randomized controlled trial, 120 patients with Grades 1 or 2 hemorrhoids were enrolled from November 2024 to February 2025. Participants were assigned to receive either MPFF alone or combined with Aescin. Treatment lasted 7 days, followed by a 2-month maintenance phase for those achieving hemostasis. The primary outcome measured was the recurrence of bleeding at 1 and 3 months. Results: Both groups showed no significant baseline differences. The MPFF + Aescin group had no recurrences at 1 month and a lower rate at 3 months (7.02%) compared to the MPFF group (23.52%, p=0.016). This combination also improved quality of life, with lower SHSHD scores and no reported adverse effects. Conclusion: The combination of MPFF and Aescin significantly reduces bleeding recurrence in acute internal hemorrhoids, suggesting it as a first-line treatment option. Further research on its synergistic effects is warranted.
