An association between CLDQ-NAFLD and brain-derived neurotrophic factor in Japanese patients with NAFLD

Backgrounds: NAFLD patients complain of non-specific symptoms including fatigue. Brain-derived neurotrophic factor (BDNF) is known to regulate subjective symptoms. We aimed to investigate 1) symptoms using Chronic Liver Disease Questionnaire (CLDQ)-NAFLD, a disease-specific patient-reported outcome (PRO) and 2) an association between BDNF and CLDQ-NAFLD in NAFLD patients.
Subjects: We enrolled 53 NAFLD patients (age 58 years, female/male 22/31, BMI 26.0, liver stiffness 5.9 kPa). PRO was assessed by CLDQ-NAFLD (minimal point=0, maximal point =7) consisted of 6 domains (Abdominal Symptoms, Activity/Energy, Emotional Health, Fatigue, Systemic Symptoms, Worry). Serum BDNF level was measured by ELISA. An association of CLDQ-NAFLD score and factors was evaluated by multivariate correlation analysis.
Results: 1) The total CLDQ-NAFLD score was 5.7 (5.3-6.1) points and fatigue was the lowest-ranking domain (5.0 [4.3–5.6] points).
2) There was no significant correlation between CLDQ-NAFLD score and age, liver stiffness, hepatic steatosis, ALT level, and HbA1c value. However, CLDQ-NAFLD score showed a significant correlation with BDNF level (r=-0.5759, P=0.0250). Moreover, BDNF level showed a significant correlation with Fatigue domain of CLDQ-NAFLD (r=-0.4883, P=0.0289).
Conclusions: We demonstrated that 1) fatigue was a frequently impaired PRO, and 2) BDNF was associated with fatigue in NAFLD patients. Since fatigue is related to physical inactivity, alteration in BDNF may be a crucial pathology for NAFLD.
