Cytoplasmic TERT expression is closely associated with hepatitis B viral infection and DNA-PKcs expression in hepatocellular carcinoma patients undergoing liver resection.

Background: Previously, we had reported that the novel biomarker, DNA-dependent protein kinase catalytic subunit (DNA-PKcs) in hepatocellular carcinoma (HCC) was correlated with Hepatitis B virus (HBV) infection and postoperative outcome. Telomerase reverse transcriptase (TERT) plays an important role in carcinogenesis among HBV infected liver. However, the relationship between DNA-PKcs and TERT, and intracellular distribution of TERT in tumor are still unclear.
Methods: Expression of TERT and DNA-PKcs were examined by immunohistochemistry among 135 HCCs. The assessment of TERT expression was assigned as follows; stained nuclear or cytoplasm of tumor above 30 %: positive. The assessment of DNA-PKcs expression was scored as follows; stained nuclear or cytoplasm of tumor above 30 %: positive. The relationship between TERT expression and clinical characteristics as well as patient outcomes was investigated by statistical analyses.
Results: Among the 135 HCCs, 86 tumors had only cytoplasmic positive, 3 tumors had only nucleus positive, 5 tumors had both positive and 41 tumors had both negative. Cytoplasmic TERT expression was significantly associated with HBs antigen, poorly differentiation and DNA-PKcs expression. No significant differences in overall survival and relapse-free survival were observed between tumor with cytoplasmic and nucleus TERT expression.
Conclusions: TERT expression in HCCs was frequently distributed to cytoplasm. Cytoplasmic TERT expression is closely associated with HBV infection and DNA-PKcs expression.
