Diagnostic accuracy of NBI Expert Team classification for colorectal tumors in our hospital.

Background and Aim: Adequate colonoscopic diagnosis before the treatment is the most important method. Several classifications about NBI magnifying endoscopy were aggregated to Japan NBI Expert Team (JNET) classification. In this study we studied about the usefulness of this classification before the suitable treatment using laparoscopy or endoscopy for the suspected submucosal invasive cancer. Method: 2637 lesions of 1637 cases except for advanced colon cancer in one year between January and December, 2016 were diagnosed before the treatment according to the criteria issued by Digestive Endoscopy(2016), respectively. All lesions were observed by magnifying endoscopy (CF:HQ290 or PCF:H290, Olympus company Co., Tokyo Japan). Results: Type 1 was 188 lesions (7.1%), Type 2A was 2279 lesions (86. 4%), Type 2B was 134 lesions (5.1%) and Type 3 was 36 lesions (1.4%), respectively. All lesions shown to type 2A were remained intramucosal lesion. In contrast, 31.3%(42 lesions) of type 2B lesions were invaded into deeper SM cancer(SM-T1b). And 97.2%(35 lesion) of type 3 were shown as SM-T1b. The sensitivity, specificity and accuracy of JNET classification in our facility accordingly; 69.9%, 99.9%, and 96.9% in type 1. 96.6%, 75.3% and 93.6% in type 2A. 29.9% 97.9% and 90.6% in type 2B. 45.5%, 100% and 98.4% in type 3. Conclusion: Colon polyps shown as type 1 and 2A are indication for endoscopic treatment. In contrast to type 3 lesions are suitable to select the laparoscopic surgery. However, lesions shown to type 2B are carefully to diagnose and decide the method of treatment. Therefore, it is recommended to use the crystal violet staining by using magnifying endoscopy prior to the decision of the treatment.
