Single institutional experience of totally robotic intracorporeal anastomotic procedure using robotic linear stapling devices after gastrectomy

(Background) The da Vinci surgical system (DVSS) has been accepted and this robotic system facilitates precise dissection and movement in a confined surgical field with impressive dexterity. In 2009, our institution introduced robotic gastrectomy using DVSS and carried out more than 300 cases with both early and advanced gastric cancer. Since initial case, we completed robotic alimentary tract reconstruction procedure using linear stapler controlled by assistant surgeon. However, technical difficulty of cooperation between robotic surgeon and assistant was remained. In 2016, robotic technology has developed and robotic linear stapling device was introduced as one of the EndoWrist.(Method) A single institutional non-randomized retrospective study was performed between May 2016 and January 2017. During the period, 42 patients diagnosed gastric cancer by preoperative examinations were performed robotic gastrectomy. All of these patients were reconstructed using robotic linear stapling devices.(Results) We experienced 6 cases of esophago-jejunostomy, 18 cases of gastro-jejunostomy and 18 cases of gastro-duodenostomy. In these cases, we have no experiences of anastomosis related complications. (Conclusion) Totally robotic alimentary tract reconstruction procedure using robotic linear stapling device is feasible and safe though the case number is small. In the session, we also present technical tips of these method using video clips.
